Cardiovascular Surgery Integration: Taipei City Hospital Provides Trans-branch ECMO for Critically Ill Patients

In recent years, both the Ren’ai Branch and Zhongxiao Branch of Taipei City Hospital have been outfitted with open heart surgery equipment and medical teams, and now possess the capacity to cope with open heart surgery and to provide critical care medicine. Trans-branch integration of cardiovascular surgery was actively promoted at the beginning of the year. In the year 2015, between January and November, the trans-branch ECMO (Extracorporeal Membrane Oxygenation) team was dispatched 13 times to perform ECMO for critically ill patients, including 1 case at Zhongxiao Branch, 5 cases at Heping Branch, 4 cases at Yangming Branch and 3 cases at Zhongxing Branch, respectively.

Lu Yan-zhou, director of the cardiovascular surgery division at the Ren’ai Branch, points out that out of the 13 patients placed on ECMO, 7 suffered from cardiogenic shock while 6 suffered from adult respiratory distress syndrome. About 30% of these patients subsequently achieved a stable recovery and were taken off the ECMO.

The emergency support range comprises critically ill patients in the cardiac catheterization room and intensive care units, who suffer from cardiogenic shock, pulmonary embolism, adult respiratory distress syndrome and other life-threatening diseases. In compliance with medical policies, the emergency ECMO team will provide support for major trauma and severe pulmonary infectious diseases. In the future, the ECMO team will comply with the policy orientation of Taipei City Government and Taipei City Hospital, and prepare for possible terrorist attacks and major trauma cases, by including vascular surgeons and technicians in the team and equipping itself with autologous blood salvage equipment, to provide timely life-saving support in the operating rooms of various hospital branches.

With the prevalence of emerging infectious diseases, should critical cases of pulmonary infectious diseases similar to SARS or MERS surface in the future, the emergency support team of the cardiovascular surgery division at the Ren’ai Branch of Taipei City Hospital will also provide ECMO support at other hospital branches as a last resort therapy for critically ill patients.
